Your first visit to Sussex Chiropractic will help you determine whether chiropractic care is right for you. Dr. Loftus will do an extensive case history examination of you to ensure that your condition warrants the natural healing benefits of chiropractic adjustments.
No-Charge Consultation
Get to know us with no commitments attached. A consultation with Dr. Loftus is always FREE. If the doctor sees fit, he’ll go through your health history, provide an exam, and take X-rays, if needed.
Review of Care
Dr. Loftus will review his findings of your exam and discuss a proposed schedule for care. Your treatment will begin that day as well as any therapy treatments that your current condition warrants.
Your Time Is Valuable
At Sussex Chiropractic, we understand you have time constraints. Our staff is always prompt when it comes to your appointment. We hope to see you soon and help you stay healthy.
Forms to Print, Fill out and Bring on Your First Visit
We ask that you bring these documents on your first visit:
- Your current insurance card, as we are required to make a copy for our records, and
- Applicable payment for initial services (insurance co-pay / deductible, etc.)
